A sale could go through above your limit if it was one ECN preferencing another. Steve Goldman's Trading Desk thread covers these topics very well.

No online broker will actively seek the best trade for you. You need a good broker (hard to find) or access the ECN's yourself. But I have heard that getting those good executions is not as easy as you would think.
