Think of the Shuttle disaster. I think they knew what was going to happen before they died. It is the horror that makes people want to make jokes. If someone dies of a heart attack in their sleep- we say "Wow, not a bad way to go." Not many jokes to make about it. Of course if someone dies screwing one can make jokes- but they are all tempered with the "What a way to go" sentiment.

It is the ways we would NOT want to go that give rise to the sick jokes- precisely because they are so disturbing. No one would like to be killed in a way that allows one to contemplate death right before they experience it with the certain knowledge that it will occur. Hence the jokes- a way to release the steam of despair, and a way to contemplate the unthinkable and the horrible.
