People are not created equal. Not everyone gets to get an A or B in school, not everyone gets to be the top scorer on the team, and not everyone get to pay the same for insurance. the point of insurance is risk sharing. To pay in at different levels is a whole other ball of wax, than being denied potentially lifesaving benefits.
