They charge quarterly, and I don't think I've seen the charge to my account yet so I'll assume it's at the end of the quarter. The $1500 is the minimum charge, so I wouldn't expect any refunds. My understanding is that it is combined, so if you trade more than one account the $1500 covers all of them. If the combined account values exceed $150K, the charge is 1% of the account values.

I have a separate account with some funds, but I am not trading that myself. I really haven't paid much attention to the load fees, but I haven't seen anything explicitly like a fee. I assume the purchase price could include some loading. I don't intend to be changing these much, so it's not a big consideration for me.
