Where do these enormous amounts reflect in the balance sheet? Are they CB sales? Or, in the circumstance that a miner borrows them and then sells them, in Mining production? Or, are they sold both by the bank AND by the miner?

I agree with you about the zero sum game in that what has been sold forward today can't be sold tomorrow. In the circumstances, provided the forward selling has been absorbed, the situation is then bullish.
