To add to what Frank said (I can't believe I'm being so bold as to clarify something written by a guy in the semi biz) ...

If any other company had a track record of producing great ASICs that could take advantage of CDMA in a demonstrative way that is as good as what Qualcomm does, there would be no high switching costs. The issue is that apparently no one has really done that. So it's not a matter of high or low switching costs. It's a matter of not being able to switch at all, which is even better than having a high switching cost.
