The main HMO that sends me patients, which is Tenet run, is now beginning to capitate us specialists. I'm an ophthalmologist. I've already had run ins with the Polly Annish woman who owns the practice about staying late to see patients or coming out for emergencies. I just tell them to go to the ER. I figure the more inconvenient it is for the patients the sooner they'll get the message that they're some real sacrifices with managed care.

But Polly Anna will come around when we're capitated. Why bother if you don't make any more money?

It is interesting how adept the insurance companies and government are in exploiting what I call doctor's "teacher's pet" mentality. We have been raised to be so compulsive, but this is now working against us. I'm telling my doctor buddies it's time we start being the class cut up, i.e. militant unions, white coat flu's. We are no longer being rewarded for compulsiveness; we're being penalized.

I often think of the perverseness of the whole set up when I realize that executives in private companies get stock options for profitability. We are penalized. The Tenet Run HMO I mentioned doesn't even give us the hold backs anymore.

And I really feel sorry for you guys in academic medicine. You're supposed to grub for bucks in private practice, teach, and do research which is obviously impossible. If UCLA is anything like the medical schools here then it's dropped all pretenses of being a referral center & is now out there making a hard sell as a primary care provider. I'm even afraid to refer patients to the medical schools because they tend to squeeze every cent out of them. It's disgusting!
