Dulcamara is the quack doctor, they have him come in in the hot air balloon like a snake oil salesman in an old west carnival show. They have Belcore come marching in with a large troop of soldiers dressed up in really fancy uniforms with lots of plumes etc.

The Met puts on very elaborate productions with lots of extras. In Aida during the scene where the Egyptian army returns from conquering the Ethiopians they have have a huge parade cross the stage including horses, camels etc. There are a couple of hundred people on stage at one point.

Also what the Met does for large crowd scenes is allow 'supernumiraries' to fill out the crowd scene. These 'supers' are fans that they dress up as characters and put out on the stage. I was onstage for a performance of La Boheme with Pavarotti. Lots of fun.
