Check out the EE Times market share numbers.

VxWorks has 25%, pSOS has 11%, roll your own has ~40%, and others have 24%. There's plenty of competition, although it will probably take the DOJ 6 months and a bunch of Hart-Scott-Rodino extensions to figure that out.

Companies roll their own for a variety of reasons, especially if they have their own engineering staff specialized in embedded apps.
