I think there are a few points worth noticing here. When in college, did you lead a "hand to mouth existence?" Obviously, you hand has turned into a mouth, and it has shiny things (teeth) inside (thus the dentist). Your recent manuscript rejection is bothering you (your writing hand is not permitted to speak) -- you feel your lips are being manipulated (the strings attached). Any more details available?
