go4it, I don't know how that works, but I don't think you contact Maxam, a transfer agent does it. I suspect (a guess) that your broker will send them to you. We should know when the shares are being distributed. Contact your old broker when that occurs. However you may want to check with them now about the procedure. If it's different than my guess, let me know.
