My recollection from the last time I took a look through the Code of Federal Regulations, which isn't something that anyone, even a lawyer, ever does willingly, is that the patients don't have to jump through hoops. But I haven't looked at it since 1996.

I agree about methadone being terribly politicized. The resistance to using effective narcotics for patients is probably partly fueled by the fact that some health-care workers do abuse drugs, and some physicians do supplement their honest income by acting as "Dr. Feelgoods." But I think that the Puritans among the regulators unfairly use the bad apples as justification for their own anhedonic impulses.
