When my Grandpa was in his late 80's, he had his legs amputated. He had gangrene and wasn't really expected to survive, but the genes in my family are peculiar, and he carried on until he was almost 96. He was in constant pain, though-- "phantom pain," they call that. He rubbed his stumps from morning to night, moaning or weeping. The family begged for painkillers for him -- something better than aspirin -- but were unsuccessful, so he lived in pain for years. The reason given by his doctors for the refusal was that "he might become dependent because the pain is chronic." My mother screamed at one doctor, "So WHAT!! Why does it matter if a 90 year old man becomes dependent on a painkiller?!", but to no avail.
