1. Paramedics generally do all possible for persons in need, but I'm pretty sure all deaths officially occur at the hospitals if a coroner and/or police officers are not at the scene. This for liability as well as red tape reasons. Thus, Suzanne could very well have been dead, but still "warm", (I hate to say that.)

Excellent point! Do you know if all police departments and paramedics have a standard set of operating procedures about declaring people dead? Might such a source be on-line?

2. Knowing the killer and facing him/her. Not necessarily. If she had no reason to distrust the person, she could have been facing away, or even beginning to walk away. The killer acted out of premeditation, or instant anger (possible defense case) for whatever reason.

The argument I was trying to make was that anyone who abruptly gets out of a car (i.e. at any point prior to their final destination) probably is pretty mad at the driver. I agree that in no way does that mean you automatically think they will chase after you and stab you, but I think there's a high degree of likelihood you might turn around (even instinctively) and say "go away", thus making it somewhat likely you will be able to get your hands up at some point during an attack.
