Dan: I do not have the time to respond to all the points you made in your long post, but at least, let me rebut a misconception about CDMA. The fact it uses all the bandwidth available does not mean it exploits it in a spectrally efficient manner. It spreads the signal across the entire spectrum, but its bits/Hz transmission rate is not tailored to the specifics of each frequency band, i.e. in some bands you can transmit more bits/Hz, and in others less. In contrast, DMT comes closes to implementing the ``water-filling' channel capacity optimum allocation of power across the available frequency bands.
