I finally got an answer from TD on this today. They say that one can get a form T-2033 from any TD bank branch and complete and fax it to 416-413-3128, to the attention of "Alam", and the transfer will be effected within 3 business days. I'm going to get the form and confirm with Schwab that they can follow it on their end.
