Maurice, I've thought for some time that volume sales for these phones in the US would be as "car phones." The largest pool of people here needing this phone is probably made up of those who travel into and out of cell coverage regularly, and don't want to be out of touch whatever the reason. Now, one doesn't often travel into and out of cell coverage on foot - hence it's most often needed while motoring. The problem I see is that the car kit is way too expensive. Make it a freebie, or at least an insignificant part of the package cost. Give people a convenient way to make use of the phone in the manner many of them will see its usefulness. Or to put it in your terms, make the scarce resource more appealing, while promoting the use of minutes.
