I collected from MacDonald's when my wife broke part of a tooth.

Go get an estimate from a dentist and ask the company representative to let you fill out the form yourself that they bring. If you just answer questions what you say may get written down in some way that you did not intend.

I would just insist that the company pay for the best possible tooth restoration because if they hesistate you can always say, "Please notice that we have not added in personal disfigurement and pain and suffering" (in a letter, with a "cc" at the bottom naming a law office.

Of course, a litigiously-minded person will give you different advice.
