I think you're oversimplifying things. What I learned in law school is that a judge may be considered legally biased if he has a personal financial interest in the case or a personal relationship with one of the litigants that would make impartiality difficult to achieve. Strongly-held opinions or even preconceptions about a case do not amount to legal bias.
