Query about double booking.

I understand what happens and why it is a problem. But would not the companies that were subject to this problem have attempted to ameliorate this problem? An obvious solution would be to have a penalty for late cancellations, or increasing it. Does the industry use the same old standards with contract bookings that it used 2-3 years ago?
