I live in a third world country where health care is thoroughly capitalized. If you have money, you can get pretty good care.

Perhaps. The Philippine gov't is fascist. That is, they allow ownership of property but the State can regulate it. Those people you see dying in the street are the result.

The State can create poverty, it cannot create prosperity.
