Gotta theory on that. They are put in with a pattern and as they work loose with time new ones are added not using the same holes over.

We thought so, too, but we looked at the ties, and there were no empty holes in the ties under the metal doohickies that the spikes go into [rectangular plate that holds the rail]. If a spike came loose, wouldn't there be a hole where it used to be, that you could see when you looked at the metal plate?
