I have taken a manila folder and written on the tab "Edwarda". I am printing out what others say about her condition and putting it in the folder.

When you've collected everything, you should made a very consise list. The doctors won't care who she told this stuff to, or whatever else may have been in the message. Just in what she said, and the date on which she said it, if the poster can remember.
