In some places (like Hawaii), the poor get subsidized coverage (open to all uninsured through QUEST which has subsumed Medicaid) while employers are required to insure their employees (and most provide coverage for workers' families which are otherwise eligible for QUEST). Because of a very high degree of insurance coverage insurance is relatively cheap. Community rating prevails. Pre-existing conditions are covered at no extra charge. As you might expect FORBES thinks Hawaii is a red republic.
