Robert, One other possibility will be that each neighborhood in the industrialized world set up their own telephony, internet, data neighborhood LAN. Then they will connect this either HFC or ADSL LAN into a WAN backbone owned by a major telco, like GTE fiber backbone.
There might instead of being a few players,a lot of little tiney players, smaller than ISP's. Disney in their new village called Celebration in Orlando, set up their own network, which is hooked into their own school system, and police station. They then hook this into a backbone, this may the way in wealthy countries. I could set up and ADSL alarm line, then hook it into a major line somewhere maybe in the RBOC CO.
