If you think any GOVT agencies aren't culpable in any deaths just remember the Challenger Disaster, launched by a govt agency. Also, you are well aware of the deaths that government mandated airbags have caused, even after they were warned of the potential risk.

Govt agencies are culpable and are subject to lawsuits. Any idea who built the Challenger space craft. Hint: Not the govt.
Also how about culpability in mass deaths, by big corporations like..

Bridgestone/Firestone
Ford
TWA
USAir
Alaska Air
Exxon (Darn!! I almost forgot that one)
Union Carbide (A spill in India, killed about 4000 people and caused permanent damages to tens of thousands. Did not follow any safety procedures. US based CEO still not willing to face a fair Indian judicial system and is a fugitive)

As you can see, the list can be endless. OTOH, FAA has done a tremendous job in bringing Airlines and Boeing into safety compliant processes.
