Jon, When I applied it was online, but it was awhile ago so I really don't remember if the link was via their site or an email letter. As far as email I kept my old email address for about two months after this free dsl was operational - just in case! That was through a regular dial-up provider. Since you are going from dsl to dsl, I'm not quite sure how you would do it. One thing to remember is that when I got the service, I was one of the first, so it was all new. I have an Etrade contact for you that I will PM you since I don't think it would be too cool to post names and phone numbers. Let me know how you do.
