I think you're making it too simple.

If Lucent had offered one share of Avaya for every share of Lucent and Avaya had started trading at $1.91, the cost basis for Lucent should be the same as it is the way they did it. Your formula would make LU worth $0 (or 1/2 * Old LU cost if you meant 13 rather than 12 in your formula).
