Thanks for the comments. Only one minor quibble to add here, re: historical accident that left Europe in its present wireless position: wasn't that a failure to implement American-style intervention (deregulation)?
While de-regulation and the 1996 Telecom Reform Act have had some impact, my prior post keyed off the implementation in the US of "universal service" provisions of the FCC mandates to industry. This is the crucial difference between the PSTNs of the US vs. most of the rest of the world. As regards consumers, we didn't allow the telcos to charge by the minute for local service, though they would have loved to do so. So, I don't see deregulation as the key driver in the anomalous uptake of the Internet in the US, it was mandated cheap phone access charges that allowed the Internet to blossom. We're so far ahead in fixed line Internet connectivity basically because it is so artificially expensive elsewhere. :)
