2. Those senior citizens can sure figure out the BINGO card easily. This was not brain surgery. A little attention to detail would have guaranteed proper selection.

I actually went to a charity bingo event once with a friend. The pace of the game was said to be rather pokey compared to REAL old-folks bingo, but nonetheless, both me and my friend found we had overlooked marking out a few numbers on our multiple grid card. Neither one of us are particularly stupid or senile, its just that we were performing a task that was novel and presented in a format that we had little experience with, even though we had seen it before and had a complete theoretical knowledge of exactly what we had to do. Just to compare, the guy I was with routinely and with 100% accuracy uses electron impact mass spectral data to identify closely related organic compounds.

The point I am trying to make is that a complicated task may seem trivial after lots of experience, but even a relatively easy task can be daunting if it is novel, there is a long line outside, people are bitching about how long the old farts are taking or other pressures real or imaginary. Sure, attention to detail will eliminate most errors, but a lot of errors can be prevented from ever happening with well designed forms and equipment.
