My ballot went into a sealed envelope, with signature across the flap, all inside an outer sealed envelope. So the answer is no, no one can see anything on the outside envelope, including my name.

I believe there was a return confirmation slip, but I don't remember exactly as I didn't mail it.

Either way, there's no way of knowing if it was tampered with en-route, as you're not there to inspect it when opened.
