The very first fiber strands were laid to replace PCM in the inter-exchange trunks. It was meant to work un chunks of 1.5Mbit/s (2Mbit/s here). It is not a technology that was developed for access.

Extending SHD/SONET right to the users does not solve his problem. You can extend HDSL over a pair of wires with the same result.
