Before anyone (other than election officials) knew how any double-punched ballots existed,
But not before the democratic poll watchers noticed the confusion in the voters. What do you think a poll watcher does? They often ask people who have just voted How are you, did you have any trouble. They also check to be sure that the station isn't running out of ballots, or that a goon squad isn't turning people away at the door. When they hear of trouble they report to the democratic phone banks and coordination centers. The Republicans do this too.
