Contact Office Principal at office where you placed the order. Do it diplomatically via telephone and follow it up in writing. Make your case plainly, rationally and provide documentation. Plainly state your intentions to take this to arbitration if not quickly resolved. Ask for the home-office address and name of CEO of firm. Let office manager know that CEO is next step if not resolved quickly. Vast majority of cases are solved at this level if handled calmly. If need be then proceed up ladder of command until arbitration. The brokerage does not want to go to arbitration! good luck
