FINGERPRINTS and CHILD ABUSE INDEX CLEARNANCES are required for all members
of your household who are 18 years and over. The State now utilizes the LiveScan process to
complete these requirements. Complete the LivesScan form and call the number provided to
make an appointment at your nearest Sylvan/Identix location. The fee is $29.00 per person
and includes both DOJ and FBI clearances.
