If you are using "hardware" synonomously with firmware, I suppose that is correct.

Indeed. In addition to manner of ATR (T=0 and T=1), there is a complete Operating System (OS) mask in ROM.

PC/SC specifications can be implemented in various operating systems, and for the most part are.
