Most of the information that this person is asking about is negotiated with issuers/interested parties on a case-by-case basis and treated quite confidentially.

With regard to the shell and reverse merger rates, I believe this site (or the information package they email to anyone wanting one) has a few examples with numbers...

For the rates that investor relation firms charge, he/she might do well to start by browsing these listings, but again: few (if any) of these companies post actual prices or deal terms on their websites...

Finally, with regard to the items negotiated when dealers agree to make a market in a particular issuers' stock...I'd have to defer that one to Gene.

I'm not really sure about the specifics, though it makes sense that the negotiating power would tend to be on the issuers' side for a liquid, listed/NASDAQ issue and on the dealers' side for an OTCBB/Pink Sheet issue.
