Don't know about the rest of the world, but Canada, particularly the Province of Ontario appears to be rolling it out cheaply,...and Ma Bell Ontario doesn't price her services to lose money.

This rollout is now being extended to small population centers (less than 15000 people) more than 100 miles away from major centers.
