That is exactly what has to happen when you do an emergency blow for fun or practice. Obviously if you are doing one because of flooding in the engine room you don't care what you run into as long as it is on the surface.

When practicing you need to clear your baffles, go to periscope depth, flip up the radar mast, ensure that there is no traffic in the area, go back down and have a ball.

This is why I am saying it will be laid at the feet of the OOD. Dude didn't do his job, plain and simple. I can promise you that it is not a good career move to bump your boat into anything but the pier while the Navy has entrusted you with a part of their fleet. At least 2 people will be in charge of a waste barge in Antarctica until their discharges have been approved.
