James, My guess would be that, since most of the poor people of a small village could not pay for anesthetic, that the doctor took great pleasure in putting a gringo through the same pain. Kind of a status equalizer.
Be that as it may, I loved the simple people of Mexico. By simple, I mean that they refused to let their lives become complicated. There was always manana.
Many years ago, when other family members stayed in the fancy hotels of Acapulco, I would rent a jeep, or VW thing, and drive up to a sleepy fishing village called Zewatenejo(sp?) where the people would accept me as one of their own, and the peace was beautiful. Then after a period of time of not going to the Pacific coast, I was about to do the same thing, and a friend of mine from Mexico City told me not to waste my time because the village had been surrounded by a tourist mecca called Ixtapa. What a pity.
