Mike, I agree that US West is shockingly irresponsible to what would normally be "customers" to any other company. In this current "monopoly status granted by the State" situation, the people who pay US West are more like prisoners without recourse. If you ever contemplate moving into a brand-new subdivision within US West jurisdiction, ask homeowners who have already moved in to the subdivision if they have phone service. If they don't have phone service yet, ask how long they have been without. The answers may shock you. I lived in a brand-new house for 2 months with no phone service. And there wasn't a darn thing I could do about it except file a complaint with the state, and become another statistic.

Almost forgot what I was going to say: The US West URL Derek provided worked like a charm for me. Maybe you just hit them at the wrong time. I'd try it again. Make sure you type in both the area code and your tele# prefix. Sorry if that sounds insulting, but I can't figure anything else why the page didn't work for you.
