A physician will occasionally submit a proposal for funding for what becomes known as a "physician-sponsored IND".

Yes, it's tough to get NIH funding, but the level of "tough" varies from one NIH department to the next.

Is this sort of thing rare? Not particularly, but, again, I'm not clear what you're really inquiring about.
