Yes, there is. I do have first hand knowledge. The last fortune 100 high tech companies I worked for are still doing it. The window is Sunday Morning 1:00 am est for a duration of 2 hours max. However, the network I supported was never down completely. Sections of them comes down for new software loading or making hardware changes (yes, including adding or removing disks/controllers)

May be I should clarify myself. Normal operating environment is anytime outside of the window I mentioned above.

I said hardware that are used in mission critical applications do not fail often, i.e. availability of 99.9% like you said. The reason I brought this up was to argue on your point 'A 3-second LIP being unacceptable because of hardware failure and/or during adding/removing hardware from a FC loop.' The problem you mentioned was operation problem, not technological limitations of FC for applications such as streaming video.
