Why would a TSR program not close properly when I turn the computer off manually after the defrag but close properly the rest of the time?

I don't know. Just a SWAG (stupid wild a** guess). Windows Me has quite a few things that are known to inhibit a proper shutdown but if your normal shutdown is not causing a problem I wouldn't worry about the Enditall thing.

As far as shutting down at night is concerned, it's 6 of 1 and ½ dozen of the other. Some people contend that leaving your computer powered on wears the components out faster and others say the jolt and power surges associated with shutdown and startup do more damage. Energy usage is also a small concern to be considered. I personally shut down (roadrunner cable) most of the time but when I leave it on I use ZoneAlarm internet lock.
