As far as I can tell, the biggest impact of diagonal routing is on design. There will be some additional defect and process issues, for example I expect etching out a 45 degree corner cleanly is more difficult than etching out a 90 degree corner, but I wouldn't expect to see any significant retooling.

I don't know where they are on the reality scale. The software seems to exist already, but it isn't clear whether any diagonal-routed chips have actually been designed or manufactured.
