I'm getting the best results from my Comstock feed received by dish; very clean data, low latency and ultra-reliable... but, not cheap. The RealTick III feed also provides excellent intraday data; I've been satisifed with it for years. And, my AT Financial feed (www.atattitude.com) has been excellent, no problem there except they give you an extra daily bar every Sunday when you're trying to scan charts.

Every feed is going to have a few bad ticks but those three have been very reliable with the minimum of problems. QCharts tries hard but they've long suffered from sometimes-severe feed quality & reliability problems (strangely, their feed is received from Comstock). I also was mostly satisfied with the feed delivered by Windows on Wallstreet (via internet) but don't know how that's going now they're part of Omega Research. And, they'll swear it ain't so but their feed would bog down severely (servers get sluggish I think) in a fast market; awful to be on the receiving end of that and paying for it!

There are probably a lot of other good ideas waiting in the wings from lurkers, maybe they'll jump in here with more suggestions.
