There are still facilities in the former Soviet East, especially Kazakhstan, that would provide an opportunity for terrorist use of missiles.

I'll guess that you are not familiar with the Command and Control structure and mechasisms of the former Soviet Union. So you're unable to have any valid assessment of the viability of such a scenario.

Also, the willingness of Arab fanatics to participate in suicide bombing shows that one cannot count on even a surgical strike, taking out the responsible parties, being a disincentive.

This is not a group that uses ICBMs. They use cars, trucks and rubber dingies. There is no logical connection between the willingness to do suicide bombing and the use of a surgical strike. The use of a surgical strike is dependent on your intelligence collection capabilities. The US can spend every dime it has on an SDI system and it's worthless; use a delivery vehicle that SDI doesn't counter.

Since MAD is a gridlock situation, the use of nukes has mainly been contemplated as part of a graduated response, with the possibility of breaking off before total destruction.

Horse hockey. MAD was established as a premise in which one side could launch a full scale attack and there would be sufficient weapons/delivery vehicles remaining to assure the annihilation of the other side.
