Joe,

I would hope that a patient in very poor condition, perhaps deteriorating, would be able to request some sort of assessment, and if being given placebo, given the option of of getting the drug.

Perhaps just wishful thinking on my part -- but it would seem a more compassionate approach.

You are right though, I really have no idea if this can be done.
