bob, I am sure Henry can and will expand on this, but the short answer is "down stream" Leptin acts by attaching to a cell surface receptor (leptin receptor), but this is just the beginning. Upon leptin binding the receptor triggers a series of signals (signal transduction) mediated by a number of proteins that eventually lead to a change in the cell. Ligand's position is inside the cell. In other words Ligand's "game" is downstream in the signalling process.
