fp- true the heart is removed with the exception of a part of the atria with the larger vessel roots attached. This is true in most transplant procedures as well. LVADs are attached to the apex of the ventricle usually. If someone is at the stage where a full replacement device or transplant is indicated, it doesn't matter that they are losing their failing natural heart. It hasn't long to last and it isn't sustaining the patient's life at that point. When the heart is failing to that extent the entire body is going into a downward spiral. The metabolism even goes into an advancing catabolic state which is one of the reasons that the company warned that the first trial patients may not live very long after the implant. Their condition would be that far advanced. It is very difficult to reverse such a state even with a strong new heart. They were trying to prewarn so that there would not be a sense of hopelessness about the device should these most sick patients die. In normal practice the device should be approved , imo, to be implemented well before a patient would reach such an advanced state but far enough along in heart failure that it it is obvious such an advanced general grave state is not far off. I can tell you it was close for me getting a transplant. I was far advanced into failure and I was quite catabolic. After the transplant the main chore was to get me out of catabolism and into an anabolic state. Fortunately for me we succeeded. Apparently this first device patient is holding his own as well and believe me a strong heart, or pump in this case, helps immeasurably along with a positive attitude and some exercise as soon as possible.
